As a data subject, you hereby give your voluntary consent to the processing, management and storage of your personal data to the extent necessary for job placement services, in particular your name; surname; date of birth; address; academic degree; educational attainment; contact details (phone and/or fax number, email address); language and computer skills; and any other data indicated on this form and the attached documents in accordance with Regulation 2016/679 (GDPR) (“personal data”). You likewise confirm that the data you have provided are true.

For what purpose will my personal data be processed?

The primary purpose of the processing, management and storage of your personal data is to register the personal data for pre-contractual relations and placements (including recurrent placements) in suitable jobs with potential future employers. The purpose of processing personal data is not fulfilled at the point of the first job placement; the purpose is not considered fulfilled until the ongoing job placement activities conducted by the controller (as defined below) have ended.

Who will have access to my personal data?

Your personal data will be used for job searches by SYNERGIE Slovakia, s.r.o., company no. 35766239 and SYNERGIE TEMPORARY HELP s.r.o., company no. 48265403, both at Dunajská 4, 811 08 Bratislava, and by SYNERGIE, s.r.o., company no. 45808139 and SYNERGIE TEMPORARY HELP s.r.o., company no. 26711834, both at Na příkopě 854/14, Nové Město, 110 00 Praha 1, as joint controllers in respect of the processing of personal data (the “controller” or “controllers”).

How long will you process my personal data?

Your consent is given for a definite period of 3 years from the day your personal data are entered into the personnel database, or until you decide to withdraw your consent.

How can I withdraw my consent to the processing of my personal data?

You can withdraw your consent at any time at no charge by contacting one of the above controllers in the same manner that you gave your consent (by email or by completing the form on the website).

To whom will you provide my personal data?

Your personal data will be provided as strictly necessary only to potential employers as part of the fulfilment of the purpose of processing indicated above. Your personal data to the extent of your name, surname, address, and date of birth may also be provided to the Central Office of Labour, Social Affairs and Family in order to satisfy legal obligations in accordance with § 28b)(1) of Act No 5/2004 on employment services.

As a data subject, you confirm and give consent to the cross-border provision and making available of your personal data to third countries which may not ensure adequate protection of your personal data. By giving your consent you acknowledge that despite the maximum efforts of the controllers to efficiently and effectively ensure the security of processing of your personal data in third countries that may not ensure adequate protection of your personal data, your data may be subject to misuse and unauthorized use without any warranties or recourse. Should that occur, the controller will implement all available measures to ensure the highest level of protection of your personal data.

What are my rights in respect of personal data protection?

You have the right:

  • to ask the controller for universally intelligible information about the status of the processing of your personal data in the controller’s information system and about the source from which the controller obtained your personal data; you have the right to have your personal data rectified or erased and to a restriction of processing, the right to object to the processing, and the right to data portability,
  • to withdraw your consent to the processing of your personal data at any time in the same manner by which you gave consent,
  • to lodge a complaint with the supervisory authority.

The controllers must comply with your request to exercise your rights as indicated above and must inform you of the outcome.
